Step 2: Download and Complete Forms
To help us make your initial experience as efficient as possible, please take a moment to download our new patient information forms. You can bring these with you, email them to debbieh@siwyplasticsurgery.com or fax them to the office at '317-876-1922'.
Step 3: Initial Meeting with Dr. Siwy
With your forms already completed and forwarded to us, you will be greeted by our female only staff. We will start your consultation session in a private room. There are four steps to your meeting today and you should typically plan to spend about 60-90 minutes with us.
First, a video presentation of your potential procedure(s) will be provided that describes the surgical process, recovery time and example BEFORE and AFTER photographs.
Second, Dr. Siwy will meet with you to discuss your wants and needs, concerns and hesitations, and medical history. Working together with you, she will offer alternatives and ideas for your situation and suggest a tailored plan.
Third, depending on the desired procedure(s), Dr. Siwy will provide a private examination highlighting how the surgery would improve your being while taking measurements and calibrations that are specific to your desired body structure and new look.
Fourth, our helpful office staff will work with you to understand your insurance and financing options, if needed, and plan a surgery date that is convenient for your schedule.
